ODS Procedures for Requesting Electronic Textbooks or Course Materials:

You must complete this form in detail before submitting.
If you do not provide exact class and instructor information, your form cannot be processed by Disability Services which could result in a delay of services.


  • In order to request/receive electronic texts for courses at University Park, a student must meet with their Disability Specialist from ODS and have approval for electronic text or course material as an academic adjustment, review the Chaffee Amendment, sign the Student Acknowledgement Form, purchase a copy of any textbook or course pack for which electronic format is requested, (this is a copyright requirement), provide a receipt to ODS, sign a copy of the official policies and procedures, and complete and submit the Electronic Text Request Form.
  • Electronic Text Request Forms must be submitted to ODS according to the following schedule to ensure electronic text materials will be available by the beginning of each semester. Please check the appropriate semester and corresponding submission deadline date:

    July 21, 2008


    Forms must be submitted to ODS by one of the following methods:
    • Complete and submit the form from this web site.
    • Deliver the form to the ODS front desk at 116 Boucke.
    • Fax the form to ODS at 814-863-3217.
  • When any changes occur to a student’s schedule (drop/add/withdrawal from courses) a new Electronic Text Request Forms must be submitted.
